Cell Proliferation and TGF-β1 Expression in Fracture Callus : Influence of Stability at the Fracture Site

Callus formation is important for fracture healing, and because callus size may be dependent on the method of fixation employed, it is important to investigate the relationships between these factors in detail. In addition, fracture callus can be divided into hard and soft callus influenced by the stability of the fracture site; there has been no data on the relationship between fracture fixation and respective calluses. To investigate the effect of fixation instability on the hard and the soft callus, the morphological changes in fracture callus were studied using a rat femur fracture model. A stainless steel pin or a nylon filament was used for internal fixation, and compared this to a group without fixation. Proliferating cells and TGF-β1 expression in fracture callus were examined by immunohistochemistry. The hematoma and the granulation tissue were larger, and the hard callus was smaller, in the groups with nylon filament fixation or without fixation. In contrast, there was no significant difference in total callus among any of the groups. More proliferating cells in the granulation tissue, as assessed by PCNA immunohistochemistry, were observed in the loose-fixation groups. The numbers of chondrocytes in those groups were also large. In the group without fixation, TGF-β1-positive-cells in the cartilage were prominent in the later stage of endochondral ossification. These results suggest that greater instability of fixation causes more connective tissue generation associated with vigorous cell proliferation, resulting in enlarged cartilage tissue. However, excessive inter fragmentary movements at the fracture site suppress chondrocyte differentiation, and this is associated with predominant TGF-β1 expression.
