Results of Strut Bone Graft for Idiopathic Avascular Necrosis of the Femoral Head

The authors discuss the mechanical basis for strut bone grafting, newly developed surgical procedure, to treat avascular necrosis of the femoral head and report the clinical results obtained with this method. The two-year or more follow-up of 45 joints thus treated (38 with steroid-induced necrosis, of which 37 classified as Type I-C or II presented with a high risk of collapse) confirmed the validity of this procedure to treat Type I-C joints: the results were satisfactory in 23 (60%) of the 38 joints thus classified, with a 2 mm or less collapse of the femoral head. However, the femoral head collapsed by 4 mm or more in 10 (83%) of the 12 Type II joints. These results suggest that this operation which is simpler and faster than osteotomy is indicated in the treatment of joints presenting with Stage II avascular necrosis with very slight collapse and classified as Type I-B or C with relatively extensive necrosis.
