IFOSFAMIDE, ADRIAMYCIN AND CISPLATIN (IAP) PLUS BLEOMYCIN (B) COMBINATION CHEMOTHERAPY FOR PATIENTS WITH RECURRENT CANCER OF THE UTERINE CERVIX

9名の再発子宮頚癌患者に対しifosfamide, adriamycin, cisplatin, bleomycin (IAP plus B) 併用療法を施行した。全例, 放射線療法を受けているが, そのうち6名は増感剤としてhydroxyureaが投与されている。評価可能な7例には測定可能病変が認められた (照射野内4, 照射野外3)。組織型ではSCC5 (NKL2, K3), adenosquamous 2, adeno 1, argyrophil cell carcinoma 1例と多彩性を認めた。評価可能症例7例中6例にresponseが認められ (奏効率85.7%), その内訳はCR3, PR3であり, 平均奏効時間はそれぞれ7カ月以上と3.5カ月であった。また効果が得られ難いとされている照射野内再発癌に対しても4例中3例にresponseが得られた。主な副作用は骨髄機能障害であり, grade 4の白血球減少が必発した。しかし, 2例に輸血が必要であったものの多くは自然回復し, 4週間隔, 計5コースの治療が可能であった。以上の結果より再発子宮頚癌に対するIAP plus B療法は高い奏効性とCR獲得率を示し, CRの平均奏効期間は7カ月以上と現在も延長中であることより, 有効な治療法と判断された。唯一の無効例はargyrophil cell carcinomaであり, この特殊な子宮頚癌の治療上の困難性が再確認された。
