TREATMENT OF GYNECOLOGICAL ADENOCARCINOMAS WITH A COMBINATION OF IFOSFAMIDE, ADRIAMYCIN AND CISPLATIN

ifosfamide と cisplatin併用による作用増強効果が報告されていることから, CAP療法の cyclo-phosphamideにかえてifosfamideを使用したIAP (ifosfamide 1g/m^2×5 days, adriamycin 50mg/m^2, cisplatin 50mg/m^2)療法を考案し, 測定可能病変をもつ婦人科腺癌患者14人 (卵巣7, 子宮体癌4, 腹膜癌2, 乳癌1)に施行した. response rateは100%であり, そのうちCRは3例で平均9ヵ月以上持続している. PRは11例で, このうち2例のCAP抵抗症例も含まれた. 本療法の副作用では, 一時的ではあるが骨髄機能抑制が目立ち, grade 4の白血球減少が85.7%に出現し, その間, 最大限の感染予防対策が必要であつた. ifosfamide特有の副作用である出血性膀胱炎は, mesna(400mg/body×3)の併用ではほとんど認められなかつた. 更に, 対象の平均年齢が60.1歳と高齢であつたことや, それぞれの残存腫瘍が大きかつたことから心配された ifosfamide, mesna併用時の中枢神経障害も極めて軽微であり, 1例にのみ軽いdepressionが出現したにとどまつた. 以上より, 婦人科腺癌患者に対するIAP療法は, 効果的でありかつ副作用も容認できる範囲である事が判明した.
