A case of necrotizing angiitis in the liver associated with infection of the biliary tract.

A case of hypersensitivity angiitis is reported in which the onset was associated with infection of the biliary tract. A 73-year-old woman was admitted to our hospital in September, 1980, for fever and peripheral neuropathy. Pneumobilia was revealed by abdomen X-ray and stones were found out in the choledochus from ERCP. Treatment with antibiotics did not improve her fever.<BR>She was operated for cholelithiasis, and the liver biopsy showed necrotizing angiitis. After operation, the temperature was decreased and general condition was improved. One year later she died of infection in the respiratory and urinary tract. Autopsy was performed. Healed angiitis were present only in the liver, gall bladder, and choledochus but not in the other organs. One of the pathogenesis of necrotizing angiitis was hypersensitivity reaction associated with infection, drug and so on. This is the first case of hypersensitivity angiitis in which infection of the biliary tract is of possible etiological significance.
