Effect of Daily Consumption Dose of Ethanol on the Ethanol Induced Hyperuricemia

Although drinking a large amount of ethanol clearly increase serum uric acid level, a relatively small amount of ethanol is reported to have little effect on serum uric acid level. To find any difference in uric acid turnover between heavy and non/moderate drinkers, we attempted to examine the effect of ethanol ingestion on the concentrations of ethanol and purine metabolites. Normal men participated in the study and took a small amount of whisky (0.5g/kg as ethanol). Serum uric acid level was elevated in heavy drinkers(daily ethanol intake more than 60g)by 0.9±0.4mg/dl 180min after drinking, whereas it was not appreciably increased in non/moderate drinkers (daily ethanol intake less than 20g). Urinary excretion of uric acid was not different between two groups. Acetate is an oxidative intermediate of ethanol metabolism and requires ATP for its conversion to acetyl CoA. Blood oxypurines, degradative prouducts of adenine nucleotides and precursors for uric acid synthesis, were also increased more in heavy drinkers than in non/moderate drinkers, showing that purine degradation and subsequent uric acid synthesis are increased after drinking in heavy drinkers. We conclude that enhanced uric acid production coupled with enhanced ethanol metaboilism is important for ethanol-induced hyperuricemia. Enhancement of ethanol metabolism, which may occur after longterm ethanol intake, should be took into consideration when treating hyperuricemic/gout patients.
