Functioning free muscle transplantation is available for the reconstruction of the missed function.We report a case of severe volkmann's contracture treated by free muscle transplantation to reconstruct the finger flexion.A patient, 19-year-old male, crushed his forearm by a pressor and lost the function of the finger flexion due to Volkmann's contracture with radial nerve palsy.As the tendon-transfer was impossible, because of radial nerve palsy, we transplanted the semitendinous muscle for the musculature of the finger flexor by microneurovascular anastomoses.An electromyographic tracing of the muscle transplanted showed the interference pattern 5 months after operation.His grip strength reached to 12kg, and he returned to the heavy labor 4 years after operation.
