The radial head fracture combined with the dislocation of the distal radioulnar (RU) joint which is called Essex-Lopresti injury is very rare.We report one case of this injury. A twenty-four years old man fell down on his right palm in a motorcycle accident. He visited his local G. P. and the fracture of the radial head and dislocation of the distal RU joint was pointed out. Two days after injury, resection of the radial head was carried out but a reduction of the distal RU joint was impossible. Five weeks after injury, he visited our hospital because of wrist pain and a limitation of supination-pronation.We planned Sauvé-Kapandji's operation however, the movement of his forearm gradually improved to 80° in supination and 45° in pronation six months after injury. Therefore we only observed his forearm.We investigated seven cases which have been reported and conclude that it is important to reduce the dislocaiion of the distal RU joint as well as to treat the radial head for preserving good movement of the forearm.
