This is a retrospective study of 14 patients with Kienböck's disease treated by radial shortening from 1981 to 1985. All patients were followed for a mininum of 1 year postoperatively (the averaged time of follow up was 32 months-range, 15 to 64 months-). In most patients pain essentially decreased the grip strength recovered to near normal, and the range of motion improved. The radiographs at follow-up showed no change in the lunate bone in 12 patients, progressive change in 2 and reparative change in none. Accoording to Evans' evaluation, patients of preoperative stage III of Lichtman's classification showed better results than those of stage IV, and all patients of stage IV with radio-carpal osteoarthritis showed poor results.
