Staphylococcal scalded skin syndrome in a uremic patient undergoing maintenance hemodialysis

Staphylococcal scalded skin syndrome (SSSS) is characterized by skin lesions of toxic epidermal necrosis, bullous impetigo and scarlantiform rash. The adult form of SSSS is rare and life threatening. A 30-year-old woman undergoing maintenance hemodialysis developed SSSS with high fever and exfoliative erythmatous rash on the neck, chest wall, back and all limbs. Skin biopsy revealed necrosis and vacuolization of cells throughout the stratum granulosum. Staphylococcus aureus was detected from blood and wound specimens. She had a worse condition with sepsis and a coagulation abnormality in addition to SSSS. Intravenous administration of minocycline hydrochloride and nafamostat mesilate was initialed on hospital day 2 and erythema and exfoliation improved within days. Early diagnosis and treatment are very effective for SSSS.
