A study regarding the relationship between the complexity of heart rate variability and hemodialysis-induced hypotension

Objective : The complexity of heart rate variability (HRV) is an indicator of the adaptability of the cardiovascular system to perturbations. We previously demonstrated that the HRV complexity at pre-dialysis session was a good predictor for intradialytic hypotension (IDH). As the second step, we investigated the influence of hemodialysis on the HRV complexity and the hypotension accompanying the influence. Methods : Hemodialysis patients with (n=13) and without IDH (n=19) underwent assessment of electrocardiogram, blood pressure and relative change of the blood volume at pre-dialysis and at the end of the dialysis session. Autonomic nervous system function was estimated by the frequency analysis of R-R intervals, and the HRV complexity was estimated from R-R interval data by the entropy analysis method. Results : All variables of frequency domain of HRV were higher at the end of the dialysis session in patients without IDH than in patients with IDH (P<.05). The HRV complexity of the group without IDH was not changed by dialysis treatment, but the complexity significantly decreased in the group with IDH (P<.01). Conclusions : We conclude that IDH may be induced by an insufficient increase in HR and decreased HRV complexity during the hemodialysis sessions.
