Spontaneous Remission of Secondary Hyperparathyroidism due to Hemorrhagic Infarction in a Uremic Patient Undergoing Maintenance Hemodialysis

Secondary hyperparathyroidism (2-HPT) is one of the most important complications in uremic patients undergoing maintenance hemodialysis. A 52-year-old woman presented with abrupt onset of hypocalcemia and decrease of serum parathyroid hormone (PTH). Computed tomography (CT) of her neck revealed an enlarged lower parathyroid gland on the left. Magnetic resonance imaging (MRI) demonstrated a high intensity on T1-weighted image, and a non-uniform high intensity and round intensity on T2-weighted image, compatible with hemorrhagic changes. The serum of calcium concentration reverted to normal range by the administration of vitamin D. She remained asymptomatic at 6 months following the episode. The previously enlarged parathyroid gland decreased in volume on CT. Although spontaneous remission of primary hyperparathyroidism due to infarction has so far been reported in 30 cases, that of secondary hyperparathyroidism in hemodialyzed patients were evidenced in only 2 patients. Although glandular infarction in 2-HPT occurs very rarely, an episode of acutely developed hypocalcemia seen in 2-HPT may strongly suggest this possibility.
