ULTRASONOGRAPHIC STUDIES ON FIRST PREGNANCY FOLLOWING MOLAR PREGNANCY

The first pregnancy following molar pregnancy feared to have recurrence of hydatidiform mole or to change to destructive mole or chorioepithelioma and showes a high incidence of abortion and premature labor, not leading to normal pregnancy and labor in many cases. Recently, we have studied 25 cases which showed amenorrhea and high hCG values after hydatidiform mole. Primary ultrasonographic diagnosis showed normal pregnancy 13 cases, lower implanted placenta 4 cases, lower implanted placenta in twin pregnancy 1 case, hydatidiform mole 1 case, threatened abrtion 2 cases and incomplete abortion 4 cases. Final results showed normal labor 16 cases, of which 3 cases were lower implanted placenta (of them, one case being twin pregnancy), 5 cases spontaneous abortion, 2 cases hydatidiform mole, 1 case destructive mole and 1 case chorioepitherioma. Thus, we were able to find changes in the intrauterine echogram during a period from the initial ultrasonographic diagnosis to final results. Where hCG value was high in follow-up patients after hydatidiform mole, it is therefore important from a view point of follow-up pregnant women to make observations by ultrasonograph at an interval of 1 to 2 weeks at first in normal pregnancy or threatened abortion or incomplete abortion not to mention destructive mole or chorioepithelioma for early detection and also to make follow-up observations thereafter since abortion due to lower implanted placenta, premature labor or placenta previa are likely to occur often even in cases of normal pregnancy.
