Application of two-color FISH with liquid-based thin-layer cytologic preparations for uterine adenocarcinomas

Object:Among uterine cervical or endometrial adenocarcinoma,there are some cases that are difficult to evaluate the degree of nuclear atypia such as adenoma malignum or well-differentiated endometrioid adenocarcinoma. We aimed at detecting numerical or structural alterations in 1p or 17p, where tumor supressor genes are located,by two-color fluorescence in situ hybridization (FISH). Materials and Methods:In this study,liquid-based thin-layer(ThinPrep) cytologic preparations were made from 50 cervical cancer cases(including 20 adenocarcinomas) and 17 endometrial adenocarcinoma cases and were analyzed by two-color FISH using a combination of probes for 17cen(cCI 17-321) and for 17p13.3 (D17S34),and a combination of probes for 1q12(DIZI) and for 1p36(cCI1-5335) after obtaining informed consent. Two-color FISH was performed on Carnoy's solution-fixed slides. The centromeric and telomeric signals from each nucleus were counted. The percentage of the three categories (monosomy, disomy with loss of short arm, and polysomy with loss of short arm) was calculated, and when the percentage exceeds the M+3SD value of the three categories in specimens,we considered that 17p13.3/1p36 region was deleted. We could make several thin-layer preparations at one time. Results:FISH detected the deletions of 17p13.3/1p36 in 95%(20/21) of cervical adenocarcinomas and also even in a case of adenoma malignum,and numerical or structural abnormalities such as polysomy, monosomy,or deletion,were detected in 84%(16/19) of the cases of endometrial cancer,and in 77%(10/13) of well-differentiated endometrioid adenocarcinoma cases including 3 cases that could not be detected by conventional Papanicolau cytologic staining. Summary:We found numerical or structural abnormalities on ThinPrep cytologic preparations of cervical and endometrial adenocarcinoma.Therefore,this two-color FISH may be a useful diagnostic method even for adenocarcinoma cases showing slight nuclear atypia on conventional cytologic preparations.
