SPIRONOLACTONE THERAPY FOR HYPERANDROGENIC ANOVULATORY WOMEN : CLINICAL AND ENDOCRINOLOGICAL STUDY

男性ホルモン過剰症を伴う月経異常患者に対して,抗androgen剤であるSpironolactoneを投与し,本薬剤の血中ホルモンおよびSex Hormone Binding Globulin(SHBG)動態におよぼす影響ならびに,その臨床効果について検討した.対象は無月経または稀発月経を呈する当科外来患者のうち,多毛,座瘡,脂漏などの男性ホルモン過剰症を認めた17例である.患者群の血中Testosterone(T), Dihydrotestosterone(DHT), Estmdiol(E_2), Luteinizing Hormone(LH)はcontrol群に対し有意に高値を示した(p<0.05).Spironolactone(100-150mg/day)4週間投与により, 血中TおよびDHTは有意に低下した.Dehydroepiandrosterone-sulfate(DHEA-S), Androstenedione(A), E_2およびLHも低下傾向を示したが有意差は認めなかつた.血中SHBGはcontrol群に比して患者群で有意に低値を示し(p<0.05), Spironolactone投与によりしだいに増加する傾向を示した.Spironolactone投与により, 多毛を示した患者10例中2例に改善効果を認めた. また座瘡は5例中4例, 脂漏は8例中7例で改善または消失した. 治療経過中,9例の無月経患者のうち2例に排卵を認めた. また6例の無月経患者に比較的, 規則的な出血をみたことは, 卵巣機能の改善として評価してもよいと考えられた.副作用としては, 脱力感および多飲・多尿がそれぞれ2例ずつにみられたが, 症状は軽度であり, 投与を継続した.Spironolactoneは副作用が少なく, 卵巣機能に対しても改善効果が認められることから, 特に男性ホルモン過剰症を伴つた月経異常患者に対し有用な薬剤であると考えられた.
