ROLE OF SEX-HORMONE-BINDING GLOBULIN AND FREE SEX STEROID HORMONES IN HYPERANDROGENIC ANOVULATION

Synopsis To clarify the mechanism of hyperandrogenlc anovulation, the binding capacity of sex,hormone.binding globulin (SHBG.BC), the levels of free steroid hormones, and the levels of basal serum testosterone(T), estradiol (E2), estrone (E,) and gonadotroplns were determined in 42 hyperandrogenic anovulatorywomenThe mean levels of basal T (0 74i0 o4ng/ml, p<0 001), free T (3 07i0 5lng/dl, p<0 01) and free E2(2 26i0 27pg/ml, p<0 05), basal luteinizing hormone (LH) (33 4i2 7mlU/ml, p<0 001), responsiveness ofLH (180 7i2l 5mlU/ml, p<0 001) after lutelnizing hormone releasing hormone (LHRH) administration,and the basal LH/basal follicle stimulating hormone (FSH) ratio (3 42i0 25, p<0 001) in the patients weresignificantly higher, the SHBG-BC level (l 76i0 33,gig DHT bound/dl, p<0 05) significantly reduced, andbasal levels of E2 (38 0i3 2pg/ml) and E, (110il3pg/ml) unchanged compared to the controlsThese findings suggest that increased T in hyperandrogenic anovulatory women results in decreasedSHBG,BC and increased free T and E2 which might be the cause of inappropriate gonadotropln secretionfollowed by chronic anovulation
