An autopsy case of hepatocellular carcinoma showing an interesting growth pattern (intrastromal growth).

We report an autopsy case of hepatocellular carcinoma (HCC) that showed invasive growth into the stroma among pseudolobules. The patient, 72-yr-old man, had been treated by transcathether arterial embolization (TAE) for HCC. At autopsy, six HCC nodules (<2cm in diameter) were found. In two HCC nodules, tumor cells were necrotic, but a few viable tumor cells were found within the cirrhotic stroma adjacent to the necrotic zone. In the remaining four HCC nodules, tumor cells were viable and invading into the pre-existing cirrhotic stroma and portal tracts. These intrastromal tumor cells occasionally contacted with blood vessels, and sometimes invaded into the vascular lumina. Despite TAE, tumor cells in the stroma remained viable, suggesting that they were nourished by portal blood. It was suggested that stromal invasion of small HCC pose problems regarding tumor growth, intrahepatic metastasis and treatment.
