The treatment of trochanteric fractures using a captured hip screw.

From December, 1984 to January, 1986, 48 cases (12 males and 36 females) of trochanteric fractures were treated in our hospital. 37 cases were repaired with Captured hip screws, 7 cases by Ender methods, 3 cases by pinning methods and one case with femoral head replacement.The advantages of the Captured hip screw were ease of handling and robust fixing ability. The Ender methods were suitable for the treatment of unstable fractures whereas the pinning methods were suitable for patients with a poor general condition. Both stable and unstable fractures repaired with Captured hip screws resulted satisfactorily.However, the use of the Capture hip screw is slightly limited. If the point of the barrel insertion is proximal to the fracture line, separation of the fracture would occur because the side-plate and barrel are not fixed together. It is possible that the barrel may pass through the fracture line as the barrel length is quite adequate.We can not apply compression to the fracture because there is no compression screw with the Capture hip screw.
