A boy aged having a severe skeletal Class II jaw base relationship, was treated with straight pull headgear, a transpalatal arch and edgewise appliances. Phase I treatment was stopped because of poor compliance with wearing the headgear. Before phase II treatment, the patient had a skeletal Class II malocclusion with mandibular retrognathism and labially inclined upper incisors protruding into the upper lip. Following extraction of the upper first premolars and lower second premolars, severe protrusion of upper incisors was corrected and upper lip protrusion was improved. Acceptable facial esthetics and good occlusion were obtained. Improvements in the facial profile and dental arches remained stable for two years.
