Treatment of a Class II malocclusion with a high mandibular plane angle and severe tooth crowding : a case report

This paper concerns treatment of a female patient with a skeletal Class II and dental Class II, Division 1 malocclusion accompanied by a high mandibular plane. There were marked arch length discrepancies in the upper and lower dental arches and significant upper incisor protrusion. We extracted the impacted upper third molars and distalized the upper second molars with the help of a lingual arch and a straight-pull headgear, which was followed by extraction of the four first premolars and retraction of the canines and incisors. Class II molar relationships were corrected by differential anteroposterior tooth movement into extraction spaces. Severe tooth crowding was treated with an edgewise appliance after removing the four first premolars. A good occlusion and facial esthetics were achieved, and these results have been maintained for more than three years after completion of the active treatment.
