Virological studies on the patients with anti-HBe positive chronic hepatitis showing GPT fluctuation.

The change in HBV markers of 118 patients with chronic hepatitis type B were studied with special reference to their clinical courses. The patients were divided into 3 groups (HBeAg positive group: 85 cases. anti-HBe positive and abnormal GPT group: 16 cases. anti-HBe positive and normal GPT group: 17 cases)Serum HBV-DNA was detected in 99%, 91%, 0%, respectively. Likewise HBV-DNA-P was in 95%, 94%, 0%, and HBcAg in livers was detected in 51%, 20%, 0%, respectively. An increase of HBV-DNA and DNA-P preceeding exacerbation of hepatitis was detected in 94% of the HBeAg positive group and in 82% of the anti-HBe positive and abnormal GPT group.In 5 cases of the anti-HBe positive and abnormal GPT group, GPT was normalized in the successive peroid. However, in continuous abnormal GPT cases, 3 of 4 progressed to liver cirrhosis and 2 cases resulted in hepatic failure. These data suggest that patients with anti-HBe positive chronic hepatitis showing GPT fluctuation were related to HBV replication and that these cases should be observed carefuly.
