Clinicopathological Analysis of Colorectal Cancer with Invasion to the Adjacent Organs.

We evaluated clinicopathological characteristics and prognosis in 69 patients with invasion of large bowel cancer to other organs ( si ( ai)), and 191 patients with s (a2) as controls. Cancer was located in the cecum in 10 patients each the si (ai) group and the s (a2) group and the sigmoid colon in 35 patients in the si (ai) group and 47 in the s ( a 2 ) group. The rates of tumor with a diameter z 6.Ocm, tumor involving the entire circumference, type 3, poorly-differentiated adenocarcinoma, and mucinous carcinoma were significantly higher in the si (ai) group than in that s (a2) group, but the rate of lymphnode metastasis or vascular metastasis did not differ between the two groups. The invaded organ differed depending on the location of the tumor. The frequently invaded organ were the liver and duodenum for the tumors in the ascending or transverse colon, the bladder and small intestine for those in the sigmoid colon, the genital organs, urinary organs, and pelvic wall for those in the rectum. The cumulative survival rate was lower in the si (ai) group than in the s (a2) group. However, the cumulative survival rate in only patients treated by curative resection was similar between the two groups. The refore, in patients at risk of invation to other organs, prognosis may be improved by selection of a procedure involving lymphnode dissection in addition to resection of invaded organs.
