A case report of primary adenocarcinoma of the vermiform appendix with observation preoperative course.

We experienced a case of carcinoma of the appendix which was diagnosed by an abdominal ultrasound examination and barium enema because of signs of acute appendicitis, and comparison with findings from a barium enema performed 7 months previously at another hospital due to similar symptoms. The subject was a 50-year-old man. Ultrasonically, the appendix was swollen, and the wall showed irregular with a nonunifrom central part. In the barium enema findings 7 months previously, only part of the aperture of the appendix was observed, and the wall was stiff and abnormally extended. The cecal membrane at the aperture was oppressed upward. The barium enema at wall institute revealed a shadow defect on the right side of the cecum, which showed a stiff and irregular pattern. Therefore, carcinoma of the appendix was suspected, and the ileum area was excised systematically, including lymphnode dissection. The histopathological examination revealed a primary adenocarcinoma of the appendix of the colon type present mainly in the roof of the appendix. The diagnosis was V, type 5, pm, ly (-), v (-), n (-), P<SUB>0</SUB>, H<SUB>0</SUB>, M (-), stage I. Clinico pathological aspects of carcinoma of the appendix were also discussed.
