Kampo is a Japanese variant of Chinese traditional medicine that involves the extensive use of herbs. The name is derived from the Japanese symbols kan, which means China and po, which means medicine. Kampo treatment has become very much integrated in the Japanese health care system. Kampo herbal preparations are sold by many Japanese pharmacies. As negative standpoints of Kampo, complicated concept of sho is the first problem. Western physicians are putt off from the start if they are told that is impossible to realize Kampo without understanding the full complexities of the basic concepts. Kampo was practiced according to the traditional Sho diagnosis, putting emphasis on the symptomatic pictures and physical constitution. This diagnostic system is different from the modern concept of disease classification. All the original guidelines for Kampo application are described with Sho diagnosis. The second problem is that few of the evidence based medical research in English literatures. In parallel to the increase in clinical use, scientific basic and clinical research began to be performed, which proved significant effectiveness in various conditions. The third problem is that there were no broad significant educations of Kampo to physicians. Most physicians have little knowledge of the original Kampo theory, since they were not educated regularly at medical school. Necessary factors for globalization of Kampo are suggested to simplify the concept of sho, establish of Evidence Based Medicine in clinical research, and to educate to medeical students and physicians more systematically.
