Effect of Mexiletine on Ischemic Heart : Studies in the Closed Chest Rabbit Acute Ischemia Model

It has been reported that nicorandil, i.e., the ATP-sensitive K^+ (KATP) channel opener shows cardioprotective effect in acute myocardial ischemia. Mexiletine (Mex), a class Ib antiarrhythmic drug, has KATP channel opening effect so that it may also show cardioperotective effect. In the present study, the effect of mexiletine on acute myocardial ischemia was evaluated in a unique closed-chest acute ischemia and reperfusion model in rabbits. In 12 rabbits, the large left ventricular branch (LLVB) of the left coronary artery was occluded for 30 minutes by inserting a catheter-bead (φ0.5〜0.7×1.5 mm) through the left carotid artery under pentobarbital anesthesia and it was reperfused by pulling out the bead. The rabbits were divided into the control (n=6) and Mex groups (n=6). In the Mex group, mexiletine (24 mg/kg/hr) was continuously infused since 30 minutes before the coronary occlusion until 90 minutes after the reperfusion. The incidence of arrhythmia, hemodynamics, left ventricular ejection fraction (LVEF), and the infarct size were evaluated and compared between the two groups. The incidence of fatal ventricular fibrillation (VE) was observed in 2/6 (33.3%) in the control, and 1/6 (16.7%) in the Mex groups. The LVEF at 30 minutes after the reperfusion was lower in the Mex group (51±2%) than the control group (59±2%, p<0.05). However, at 360 minutes after the reperfusion, LVEF became 64±2% in the control, and 71±3% in the Mex groups, and was higher in the Mex group (p<0.05). The area of myocardial infarction determined by the 2, 3-triphenyltetrazolium chloride (TTC) stain was significantly smaller in the Mex group (12±2%) than the control group (24±3%, p<0.05). In closed-chest acute ischemia and reperfusion model in rabbits, Mex reduced the infarction size and improved the left ventricular function at late phase after the reperfusion.
