Left Ventricular Filling Profile in Diastolic Dysfunction on the Basis of Filling Velocity and Volume Relationship

We have reported a pulsed Doppler echocardiographic method to determine separately, early diastolic filling volume (EDFV) and late diastolic filling volume during atrial contraction (LDFV). The relationships between the indexes during early diastolic filling (EDFV and peak early diastolic filling velocity) and those during late diastolic filling (LDFV and peak velocity during atrial contraction) were investigated in fifty-one subjects including fifteen normal subjects and thirty-six patients with coronary artery disease. There were significant correlations between peak early diastolic filling velocity (E velocity) and EDFV (y=0.41x+27.9, r=0.46, p=0.007), and between peak velocity during atrial contraction (A velocity) and LDFV (y=0.38x+1.5, r=0.54, p=0.0001). E velocity correlated negatively with LDFV (y=-0.24x+39.1, r=0.48, p=0.0004). However, there were no significant correlations between E velocity and A velocity, nor between EDFV and LDFV. It has been thought that impaired early diastolic filling caused augmentation of atrial contraction which resulted in a decrease of the E/A ratio. The mechanisms of this decrease of the E/A ratio have been still unclear. If that hypothesis were correct, there should be some relationship between E velocity and A velocity, but there was no correlation between them. Our data indicated that impaired early diastolic filling caused a decrease of the E velocity which led to an increase of LDFV which influenced the A velocity. We found an interaction between early and late diastolic filling through the change of filling volume and demonstrated the compensatory mechanism of the left atrium in abnormal relaxation and enhancement of early diastolic filling in abnormal chamber stiffness.
