The Importance of The Monitoring of The F-F Interval in Surface ECG in Patients with Paroxysmal Atrial Fibrillation

Although the atrial electrophysiological properties are very important to produce atrial fibrillation, it is clinically difficult to monitor those factors non-invasively. In the present study, the f-f interval during paroxysmal atrial fibrillation (PAF) was analyzed in the ambulatory ECG recordings to evaluate the usefulness of this parameter to monitor the changes in atrial electrophysiological in PAF patients. Twenty-one patients with PAF (total PAF duration>2/24 hours with normal atrial size) were clinically followed-up for 6 months without any antiarrhy thmic drugs. Twenty-four-hour Holter monitoring was performed before and after the follow-up period, and the PAF duration and the f-f intervals during the PAF episode were evaluated. PAF duration was prolonged from 187±50 to 223±79 min (P=0.034) and the f-f interval was shortened from 0.14±0.03 to 0.12±0.02 msec (P=0.003). There was an inverse relationship between ΔPAF duration and Δf-f interval (P=0.027). In clinical PAF patients, the prolongation in PAF duration was related to the shortening of the f-f interval during the PAF episodes. The electrical remodeling was suggested to play a role in promoting the appearance of the AF in PAF patients.
