Limiting Factors in Bloodless Open-Heart Surgery in Children

To identify limiting factors in bloodless open-heart surgery in children, we retrospectively analyzed 120 pediatric patients. Between January 2000 and November 2001, 120 newborns, infants or children underwent cardiac surgery with cardiopulmonary bypass in our hospital. Their ages ranged from 1 day to 23 years old with a median age of 24 months and body weight ranged from 0.9kg to 89.4kg with a median of 10kg. There were 51 girls and 69 boys. Factors selected for multivariate analysis were body weight, preoperative hematocrit level, degree of hemodilution, duration of cardiopulmonary bypass, duration of aortic cross clamp, amount of blood loss in the operating room and within a 12-hour-period after surgery. Correlations bewteen the amount of transfused blood and each factor were obtained by multiple regression analysis. The overall transfusion-free rate was 74%. The homologous blood was used in the priming of a cardiopulmonary bypass circuit in 19 patients (16%). Blood was transfused after the bypass in the operating room in 10 patients (8.3%), and within 24 hours after surgery in 2 (1.6%). Multiple regression analysis showed that the degree of hemodilution during bypass, the amount of blood loss both in the operating room and thereafter within a 12-hour-period and preoperative hematocrit level were significantly correlated with the amount of transfused blood (p<0.01). Body weight was not a determining factor in blood transfusion. In order to minimize the need for blood transfusion and accomplish bloodless openheart surgery in children, we suggest that the degree of hemodilution as well as the blood loss after surgery should be reduced.
