Results of Surgical Treatment for Infective Endocarditis : Special Consideration on Underlying Cardiac Lesions

Between September 1991 and October 1999, 28 patients underwent cardiac surgery for infective endocarditis (IE). In 13 (46%) of 28 patients, underlying heart disease was detected. The incidence of congenital heart disease was highest (8 patients, 29%). Mitral valve prolapse was detected in 3 patients (21%), showing the second highest percentage. Rheumatic heart disease predisposed in 2 patients (7%). In patients with underlying heart disease, non-cardiac predisposing factors were less marked (2 patients, 15%), the most common infectious organism was alpha-Streptococcus (10 patients, 77%), and there were no postoperative deaths or complications. However, in patients without underlying heart disease, non-cardiac predisposing factors were detected in 10 patients (67%), a variety of bacteria caused IE, one early death (6%) and two late deaths (13%) occurred, and 4 patients (27%) developed non-fatal complications. Underlying cardiac lesions influenced not only the conditions in patients with IE but also the results of surgical treatment.
