Coronary Artery Surgery in Patients with Aortitis Syndrome

Coronary surgery was performed in 19 patients with occlusive coronary lesion associated with aortitis syndrome. Transaortic coronary ostial endarterectomy (TCOE) was performed in 9 patients and coronary artery bypass grafting (CABG) was performed in 10 patients. Aortic valve replacement (AVR) was also concomitantly performed in 11 patients for the treatment of associated aortic regurgitation. A good short-term outcome was obtained in all patients, while the long-term outcome was poor in patients treated by CABG since graft obstruction was common, probably due to extension of aortitis. Therefore, further studies are necessary in order to improve the outcome of CABG for occlusive coronary lesions associated with aortitis syndrome, including modification of the procedure or introduction of steroid therapy. In contrast, TCOE appeared to be an effective and safe technique when it was appropriately indicated. Since there are few risk factors for re-stenosis after TCOE and since aortitis syndrome generally occurs in young patients, this procedure may be more useful than CABG in view of the better long-term patency.
