An Angiographic and Clinical Evaluation of Free Arterial Grafts from the Forearm for Myocardial Revascularization in the Early Postoperative Period

The choice of bypass conduit is the most important decision made before coronary artery bypass grafting. The aim of this study was to assess the early results using the radial and ulnar artery as conduits for myocardial revascularization. A total of 61 patients who underwent coronary artery bypass grafting with free arterial grafts from the forearm with the radial (n=58) or ulnar artery (n=3) were studied. All the procedures were performed conventionally via a cardio-pulmonary bypass, under mild hypothermic conditions. Radial and ulnar arteries were harvested as the vascular pedicle from the nondominant arm. The distal anastomosis of the forearm graft was performed using continuous 7-0 or 8-0 polypropylene sutures. Follow-up coronary angiography was performed between the 2nd and 5th weeks after the operation. A complete study of coronary arteries and left ventriculography was performed in 59 patients. Fifty-three radial artery grafts and 3 ulnar artery grafts showed excellent results. The patency rate of forearm grafts was 96.6%, and the average internal diameter was 2.30±0.53mm (range 0.68 to 3.62). Early angiographic evaluation of forearm grafts demonstrated excellent patency rates and feasible diameters. The use of a radial artery in combination with an internal thoracic artery is recommended. After evaluation of the arterial circulation in the forearm and hand, if evidence of radial artery dominance is observed, an ulnar artery graft should be considered.
