Enhancement of Myocardial Protective Effect of Hyperpotassemic Cardioplegia by Vitamin E in Patients Undergoing Open Heart Surgery

There have been accumulating evidences that indicate the ischemic myocardium incurrs oxygen free radicals-mediated reperfusion injury during open heart surgery. We administered vitamin E, the oxygen radical scavenger, preoperatively to patients undergoing open heart procedures employing hyperpotassemic cardioplegia (vitamin E group), and compared their clinical results with those of patients who did not have preoperative vitamin E (control group). Data were collected from patients who had aortic cross clamping longer than 60 minutes in both groups. There were 103 control group patients (64 valve procedures, 28 CABG, and 11 other operations) and 111 vitamin E group patients (73 valve procedures, 32 CABG, and 6 other operations). Preoperative LVEF was less than 50% in 16 patients of the control group and in 35 patients of the vitamin E group (p<0.02). Aortic cross clamp time was 97±35 in the control group and 96±26 minutes in the vitamin E group (NS). Intraoperatively, the serum malondialdehyde was lower in the vitamin E group than in the control group at 60 minutes after reperfusion. Postoperatively, cardiac death ensued in 7 patients of the control group and in one patient of the vitamin E group (p<0.02). Moreover, there were greater incidence of spontaneous defibrillation (17.7 vs 74.4%; p<0.001) immediately after aortic declamping, larger cardiac index (3.5±0.9 vs 4.6±1.31/m/m^2; p<0.001) in the vitamin E group. We conclude that preoperative administration of vitamin E contributes to better recovery of cardiac function in patients undergoing open heart surgery employing aortic cross clamping and hyperpotassemic cardioplegia.
