Histo- and Cytochemistry of Gland Mucus Cell Type Mucin and Its Practical Application for Pathologic and Clinical Diagnosis

Gastric mucosa is characterized by secreting two types of epithelial mucins: surface mucous cell type mucin (SMCM) and gland mucous cell type mucin (GMCM). Gland mucous cell includes cardiac gland cells, mucous neck cells, pyloric gland cells, and Brunner's gland cells of the duodenum. SMCM is positive for galactose oxidase/ cold Schiff (GOS) reaction or galactose oxidase/ cold thionin Schiff (GOCTS) reaction, and its core peptide is MUC5. GMCM, on the other hand, shows class III type reaction for paradoxical Concanavalin A staining (PCS) and is positive for the monoclonal antibody HIK1083. Its core peptide is MUC6. Gland mucous cells mentioned above mostly contained pepsinogen and lysozyme. Recently, Nakayama et al. have cloned a new glycosyltransferase, α 1,4-N-acetylglucosaminyltransferase (α 4GnT). The α-N-acetylglucosamine at the non-reducing terminal is essential to express class III reaction and to react with HIK1083. The physiological role of GMCM remained to be clarified, but data obtained in human stomach suggested that this particular mucin plays an important role to maintain homeostasis of the surface mucous gel layer and a part of non-immunological self-defense mechanism of the gastric mucosa, especially in the mucosa with Helicobacter pylori(H.P) infection. The latter organism is not observed in the gel layer containing GMCM and does not attach to the gland mucous cells which express class III mucins on their apical surface. Expression of α 4GnT is demonstrated to increase in the mucosa showing chronic active inflammation. Recent studies carried out in our laboratory have revealed that GMCM distributes not only in the gastric mucosa but also in metaplastic or neoplastic lesions in other organs. Gastric metaplasia is frequently found in pancreatic duct, bile duct, small intestine and uterine cervix and rarely in lung. Majority of adenocarcinoma of pancreas and biliary tract contains GMCM and most of macinous cystadenoma or cystadenocarcinoma of the ovary also contains GMCM. In lung, goblet cell type adenocarcinoma is phenotypically gastric type. In uterine cervix, there is an unique tumor called adenoma malignum. Its tumor cells show minimal atypia and resemble mucous cells lining cervical mucous glands. We found that adenoma malignum is phenotypically gastric type carcinoma, and GMCM is the very useful marker to identify this lesion. Furthermore, we proposed the HIK1083-labeled latex agglutination test to identify GMCM in cervical mucin. This test could be applied for goblet cell type adenocarcinoma of the lung and adenoma malignum of the uterin cervix.
