Circulatory Arrest with Profound Hypothermia During the Surgical Treatment of Large Internal Carotid Artery Aneurysm : Case Report

45才男性の左内頸動脈瘤のクリッピングの際に体外循環、超低体温下、一時的循環停止を併用した症例の報告である。動脈瘤は未破裂で診断され、最大径22mm後外側に向いていた。コイルによる塞栓術をまず試みたが、コイルが内頸動脈内にはみ出すため、塞栓せずに中止した。内頸動脈の一時的閉塞試験では、瞬時に意識障害と右片麻痺をきたした。手術は動脈瘤周囲を十分剥離し止血を完全にした後、脱血管2本、送血管1本を挿入し補助体外循環で低温を最低19℃まで冷却し24分間の循環停止中にクリッピングを行った。術後経過順調で元気に働いている。この方法は術野の拡大、動脈瘤剥離、クリッピングが容易になり有用である。
