An adult case untreated for 7 years after loss of the mandibular left first molar

The patient was a 46-year-old female whose chief complaint was difficulty in mastication due to open bite. Seven years ago, her left mandibular first molar was extracted due to a radicular cyst. Although prosthetic treatment had been planned, she did not receive the treatment because of a personal reason. As a result, the adjacent left mandibular second molar inclined in the direction of the extracted molar by approximately 90 degrees. The occlusion changed from closed bite to open bite. Stability of occlusion at the jaw-closing position was not good due to little occlusal contact area. We conclude from this case that unstable occlusion may occur after a loss of first molar if dental treatment is not performed at all for several years.
