Severe skeletal Class III case treated with surgical orthodontic treatment and tongue reduction

Lingual frenectomy, orthognathic surgery, and tongue reduction were performed on an Angle Class III adult case with skeletal mandibular prognathism, total crossbite, macroglossia, and ankylosis of the lingual frenum. Extensive mandibular setback (R : 12 mm, L : 15 mm) resulted in a good facial profile, but caused masticatory and speech problems due to a decreased intraoral space. Tongue reduction, which was performed at the early stage of the retention period, allowed the occlusion to remain stable with little relapse during approximately 7 years of follow-up. These results suggest that esthetically and functionally good results can be obtained with tongue reduction in a case requiring a large amount of mandibular setback if postoperative adaptation of the tongue seems unlikely.
