Application of caps with brackets to titanium screws for stationary anchorage in edgewise treatment

It has been reported that titanium screws, which cause less surgical damage during implantation than implants and miniplates, are useful for stationary anchorage. This report demonstrates that the application of caps with brackets to titanium screws was useful in edgewise treatment. Titanium screws 14 mm in length, 2.3 mm in diameter, and having a cubic head, were used for anchorage. The caps with brackets were made as follows : firstly, a metal sheet was shaped to match the screw head ; secondly, the cut edges of the sheet were welded together ; finally, a bracket was welded to the side surface of the cap for palatal use or to the upper surface for buccal-side alveolar bone use. We implanted the screws in the palatal and lower premolar regions and applied the caps with brackets to the screws in an adult female with malocclusion. For the upper dentition, a palatal arch was connected to the cap and bilateral first molars, and it was used as an anchor for the distal movement of the second molars without bonding other teeth. For the lower dentition, screws with brackets in the premolar regions were used as an anchor for the distal movement of the second molars without bonding teeth, excluding molars. The treatment progress suggests that the use of caps with brackets that we fabricated for application to the titanium screws makes it possible to safely and steadily move the molars in the posterior direction without bonding all of the teeth.
