正常咬合者の顎顔面形態と咬合力の関連

正常咬合者における咬合力分布と顎顔面頭蓋の関係を明らかにするために, 顎口腔機能が正常で個性正常咬合を有する成人29名の咬頭嵌合位での最大噛みしめによる咬合力, 咬合接触面積, 平均咬合圧と側面および正面頭部顔面形態の関連を検討した.咬合力の記録には咬合力測定用感圧シートを用い, 歯列全体, 各歯の咬合力, 咬合接触面積, 平均咬合圧を分析し, 顎顔面形態は側面および正面頭部X線規格写真を用いた.結果は以下の通りであった.1. 咬合力, 咬合接触面積, 咬合圧はそれぞれ男女平均で, 829.8±347.2N, 16.2±8.0mm^2, 54.7±12.1MPaを示した.2. 咬合力, 咬合接触面積は性差を示し, 個々の歯の咬合力, 咬合接触面積は後方歯に行くにしたがい増加した.3. 男性は女性に比べ, 上顎骨の幅が広く, 下顎枝の高さが長く短頭型を示した.4. 側面顎顔面形態ではshort face typeは歯列全体, 大臼歯部およぼ小臼歯部の咬合力, 咬合接触面積がlong face typeに比べ, 有意に大きい値を示した.5. 正面顎顔面形態では歯列全体の咬合力非対称指数は9.9±10.1%を示した.以上から顎口腔機能正常者の側面顎顔面形態は歯列全体ならびに大臼歯, 小臼歯の咬合力, 咬合接触面積に関連し, 正面顎顔面形態は形態的力学的に対称性を示し, バランスが保たれていることが示された.
