Pineocytoma With Massive Intratumoral Hemorrhage After Ventriculoperitoneal Shunt : Case Report

Ventriculoperitoneal Shunt(V-P Shunt)術施行後、腫瘍内、第三脳室内に大量出血し症状の悪化を認めたpineocytomaの1例を報告する。症例は58才、男性。頭痛、歩行障害、記銘力障害、尿失禁を主訴として来院。CT、MRI画像にて、水頭症、松果体部腫瘍を認めた。水頭症による症状に対して、V-P Shunt術を施行した。術後2日目に、急速に意識レベルが低下し、昏迷状態となり、パリノー兆候を認めた。CTにて、腫瘍内、第三脳室内に大量出血を認めたため、開頭による血腫及び腫瘍摘出術施行した。組織はpineocytomaで、術後症状は改善した。V-P Shunt後の出血の報告は少ないが、V-P Shunt後腫瘍摘出術を行う際、念頭においておくべきと考えられた。
