GRANULOSA CELL TUMOR OF THE OVARY WITH SERTOLI-LEYDIG CELL TUMOR COMPONENTS (GYNANDROBLASTOMA)

Gynandroblastomaはfemale sex cord由来のgranulosa theca cellとmale sex cord由来のSertoli-Leydig cellを有するために種々のホルモン分泌能を有すると考えられるが,未だ我国における報告はなく,そのホルモン動態も明らかではない.我々はかかる1例を経験したので報告する. 症例は63歳の女性で不正***出血を主訴とし,諸検査にてホルモン産生卵巣腫瘍が凝われ手術が施行された.腫瘍は左卵巣より発生せる手拳大の充実性腫瘍で,病理組織学的には充実状,索状に配列するgranulosa cell tumorと莢膜細胞様の間質成分,ならびに一部に分化したSertoli-Leydig cell tumorが証明された.内分泌学的にはestradiol,androstenedioneまた少量のtestosteroneの生成が認められ,in vitroにおけるステロイド生合成実験では,この腫瘍で生成された多量のestradiolは多くはandrostenedioneからestroneを経て生合成されたと考えられた.
