Comparison of Whole Brain Radiation Therapy and Locally Limited Radiation Therapy in the Treatment of Solitary Brain Metastases from Non-small Cell Lung Cancer

1982年から1991年の間に我々の施設で治療した肺非小細胞癌の単発性脳転移104例のうち、原発巣の摘出が可能で、かつ脳以外の臓器に転移のない61例について全脳照射と局所照射の治療効果の比較を行った。23例(A群)に対しては局所照射(外照射又は術中照射)のみ、38例(B群)に対しては全脳照射を含む放射線治療が行われた。中間生存率はA群で83.1週、B群で30.7週(P<0.05)、1年局所再発率はA群で7%、B群で21%(P>0.05)、1年遠隔再発率はAB両群共に18%であった。放射線障害によると思われる痴呆症はA群で2例(8.7%)、B群で7例(18.4%)であった(P>0.05)。今回の検討の対象としたような症例に対しては、むしろ局所照射のみの方が適切である可能性が示唆された。
