A SENSITIVE ASSAY FOR ANTI-PHOSPHATIDYLETHANOL-AMINE ANTIBODY IN PATIENTS WITH RECURRENT FETAL LOSS

近年, 反復流産の原因の一つとして血清中の抗リン脂質抗体について研究されてきている. われわれはリン脂質の中でもとくに, Lupus Anticoagulant(LAC)活性が Phosphatidylethanolamine(PE)の会合体構造に依存することや血管内皮細胞の膜リン脂質として PE が多いことなどから抗 RE 抗体に注目し, 従来測定困難とされてきた抗 PE 抗体の測定法について検討を加えた. 今回われわれは, ELISA 法の基礎実験として PE の固相化条件の検討を行い, 酢酸を添加することにより抗 PE 抗体の測定感度を上昇させることに成功し,抗 PE 抗体の測定法を確立し得た. さらに本法を用いて反復流産患者血清中の抗 PE 抗体について検討した結果, 以下のごとき成績を得た. すなわち, LA 陽性の反復流産患者10例の非妊時血清中の抗 PE 抗体は10例中10例, 陰性荷電リン脂質に対する抗体は10例中9例に陽性であった. プレドニゾロン, アスピリン療法を施行した後に妊娠終了まで同様に抗リン脂質抗体を測定したところ, 治療にもかかわらず子宮内胎児死亡に至った4例はすべて IgG 分画の抗 PE 抗体価が高値を持続していた. 一方, 抗体価が正常範囲まで下降した6例は生児を得ることに成功した. 以上より IgG 抗 PE 抗体は, 反復流産の原因の一つとして有用な指標になりうると思われた.
