Thickness of oral mucosa during edentulous impression procedures

The compressive impression method, which takes into consideration occlusal pressure during function, is generally used when taking impressions for complete dentures. However, the deformation of oral mucosa caused by pressure has not been clarified, and use of this method is often based on the dentist's experience and subjective judgment. We attempted to clarify the effect of the oral mucosa thickness on changes in load during edentulous impression procedures. To evaluate the reliability of a clinical measurement system using the ultrasonic pulse method and the effect of differences in the probe shape and structure on the measurement of oral mucosa, we measured the thickness of oral mucosa at different load levels using a measurement system based on the ultrasonic pulse-echo method. There were significant correlations between the thickness of oral mucosa measured by ultrasonography using either a V260 probe or an M116 probe and that by the needling method (p<0.01). The correlation coefficient was higher with the V260 probe that has a delay material than with the M116 probe. Measurements using the V260 probe demonstrated that the oral mucosa was thinnest at the center of the palate and thickest in the posterolateral palate. Although the mean thickness of oral mucosa increased significantly from the center of the palate to the bilateral regions, it showed no significant difference in the anteroposterior direction. These findings indicate that measurements using the system with the V260 probe that has a delay material are reliable, suggesting that this method is clinically useful. The oral mucosa was significantly thinner at rest in the center of the palate and on the buccal ridge than in other regions. The mucosa is depressed at a load level of 14.7N during the compressive impression-taking period, suggesting that a load level greater than this is unnecessary for compressive impressions. Since the load for a compressive impression may be concentrated in the buccal ridge regions, it seems advisable to relieve these areas, where the mucosa is very thin, as well as the palatal torus region.
