In 101 patients with classical and definite rheumatoid arthritis, clinical and X-ray findings of the forefoot were examined.Eighteen patients (18 per cent) of them complained of pain in the forefeet. Hallux valgus with bunion, hammer toes with pressure sores or overriding toes were found in 38 patients (38 per cent). The majority of the deformities did not closely related to pain, but associated with Steinbrocker's stages. X-ray changes of the forefeet and hands of each patient showed the same stages in 48 patients (48 per cent).Surgical treatment for 16 forefeet with painful deformities in 10 patients had been carried out and followed up for 4 years and 10 months on an average. The results of correction osteotomy of the first metatarsus or resection arthroplasty of the metatarsophalangeal joints (Lelivere's procedure) were good or fair except for 3 cases of correction osteotomy and one of resection arthroplasty. Recurrence of painful deformities developed in the patients with poor result.
