Cubital tunnel syndrome in 3 fishermen were reported in this paper. The 3 men had been working on their boat as a bird watching using binoculars to get a fishing point for about 16-30 years.They took an extremely flexed position of the bilateral elbows on a table for 12 hours a day. Paresthesia of the ring and little fingers was caused. Regional hypesthesia and atrophy of the intrinsic muscles on the affected hands were observed at the first examination.Two of them were operated on by the method of king, and their postoperative courses were good.Another one was treated conservatively. It is possible that mechanisms of these cases can be the results from an external compression force and physiological pressure by an arcuate ligament and tension of the ulnar nerve in an extremely flexed position of their elbows.We suggest that soft pad under elbows at work may help to prevent the development of this disease. Additionally, it is better to have a free time from work, a few minutes an hour.
