Severity and prognosis of liver cirrhosis assessed by organ-reflectance spectrophotometry.

The purpose of this study was to evaluate the usefulness of the investigation in hepatic hemodynamics for assessing the severity and predicting the prognosis of 30 patients with liver cirrhosis. The concentration of hemoglobin in the hepatic tissue (ΔEr569-650), the oxygen saturation of hemoglobin in local liver blood (SO2) and the in vivo hepatic oxygen consumption (VO2) decreased significantly in patients with decompensated liver cirrhosis in comparison with compensated liver cirrhosis. The SO2 in patients who could survive more than 5 years after peritoneoscopy was significantly higher than that in nonsurvivors, whereas no significant difference was found in the ΔEr569-650 and VO2 between survivors and nonsurvivors. According to survival curves up to 90 months, a decreased survival was seen if the SO2 was less than 40%.Thus, the estimation of hepatic hemodynamics by using reflectance spectrophotometry may provide a valuable information for assessing the severity and predicting the prognosis of liver cirrhosis.
