A case of hepatocellular carcinoma with portal vein thrombosis effectively treated with VP-16 (Etoposide).

A 66-year-old man had been followed up as alcoholic liver cirrhosis for 10 years until he was admitted to our hospital because of abdominal fullness on November of 1983. On admission, he had hepatomegaly which seemed to be due to the diffuse type of hepatocellular carcinoma (HCC) with portal vein thrombosis, but no high serum level of AFP was found. Because of no operative indication for HCC, VP-16 (Etoposide) was intravenously administered for two cycles (80mg/mm<SUP>2</SUP>/day on 5 consecutive days at 4 weekly intervals). After treatment, the improvement of hepatomegaly (reduction rate 44%, Partial Response) and portal vein thrombosis were shown using Ultrasound and CT scan. He complained of nausea, anorexia and alopecia, but no other severe side effects inclusing bone marrow suppression were found. From these result, it is suggested that VP-16 may be an effective agent for HCC.
