Behavior of small and large molecular hepatitis B e antigens in patients with chronic HBV infection and its relationship with liver injury.

IgG-free small molecular hepatitis B e antigen (small HBeAg) and IgG-bound large molecular HBeAg (large HBeAg) in serum were quantitatively determined by enzyme immunoassay, respectively.<BR>In asymptomatic HBsAg carrier (ASC), small HBeAg concentration (conc.) changed according to movement of total HBeAg conc. but remarkable change was not seen in large HBeAg conc. At development of hepatitis in ASC, initially marked increment in conc. of small HBeAg was observed and subsequently large HBeAg conc. increased accompanied with elevation of serum GPT activity and simultaneously large/total HBeAg conc. ratio quickly rose.<BR>In cbronic aggressive hepatitis, large HBeAg conc. changed correlated with movements of total, small HBeAg conc. and serum GPT activity. But under corticosteroid therapy, movements of small and large HBeAg conc. were dissociated. The latter moved in accord with serum GPT activity.<BR>Large HBeAg closely related with onset of hepatitis and its activity, therefore large HBeAg may play some role in etiology of liver injury associated with HBV.
