Hepatic circulatory disturbance and reconstruction of vascular channels after obstruction of portal vein branches.

After embolization of peripheral portal vein branches, the sinusoids at the vicinity of obstructed vein increased the diameter and developed to new portal vein branches in 3-5 days. In the case of obstruction of the main and medium-sized portal vein branches, new vascular channels were constructed by recanalizaiton in 5-15 days. The obstruction of portal vein branches led consistently to hepatic infarction, in contrast with the established theory, but the infarction was restored in a short time by the reconstruction of vascular channels and the regeneration of hepatocytes. The obstruction of portal vein branches led to abrupt and marked portal hypertension and to an increase in portal vascular resistance, but these hemodynamical changes were transient and normalized in parallel with the progression of reconstruction of vascular channels. These data suggested that insufficiency of reconstruction of vascular channels is one of the important causes of persistence of portal hypertension.
