An autopsy case of carcinoma of the hepatic duct with the hepatic lobes replaced with proliferated bile ductules.

We reported a rare case of carcinoma of the hepatic duct with the hepatic lobes, the left and the quadrate lobe, replaced with proliferated bile ductules. A 72-year-old female was admitted to the Osaka Medical College Hospital for jaundice. She died of hepato-renal failure three weeks after her admission. At autopsy, carcinoma arising in the left hepatic duct invaded the left portal vein, and the left hepatic duct and the left portal vein were obstructed. The hepatic artery was not affected. The left and the quadrate lobe, which are supplied from the left portal vein, were replaced with proliferated bile ductules. The configurations of the two lobes were well kept. We suppose that the proliferation of bile ductules in the present case relates to three factors, namely continuous stimulation for proliferation of bile ductules due to gradual obstruction of the left hepatic duct, gradual loss of hepatocytes caused by reduction of regionai portal venous flow following obstruction of the left portal vein, and maintenance of the hepatic arterial flow.
